As we move through the reforms and towards the new aged care world, the practice focus shifts even more strongly to a consumer driven model based on choice and control.  What does this mean in your professional practice with clients?  How does this evolution in client practice influence the client journey from entry, through assessment, support planning and reviews, consultation and exit?  This one-hour, on-line session is designed for intake officers, assessors, care planners, care partners, coordinators and team leaders.
