Your Side Australia and the Northen Sydney Primary Health Network are collaborating to present the second of three webinars. In this webinar, participants will be building on the foundations established in the first webinar, taking a deeper dive into the major aged care funded programs, unpacking their inclusions and exclusions, and exploring how GPs and healthcare staff can use existing health system supports to complement aged care packages.

- Complementary supports available through the health system to strengthen aged care planning
- Practical approaches to integrating health and aged care supports in a coordinated plan
- Best practice in linking patients to services and making effective referrals
